We are working on survey forms to give to our grantees so they can collect information from their audiences/participants and report back to us. Most of the programs are event-based, arts & culture programs. We would like to provide them with some standards for collecting data that they can report back to us. If you have any examples of survey forms that your organization uses that you are willing to share, I would appreciate it. Thank you!
