The Flight 5191 Memorial Committee in Lexington, KY commissioned Douwe Blumberg (also see he was mentioned by Roberta Bloom) to create a memorial to the 49 souls who perished in the 5191 Comair flight departing from Lexington. The selection committee, of which I was a member, selected Douwe through an RFQ followed by an RFP process.
