Hello all, here are our surveys.  We did them on Survey Monkey and sent them out at 5pm on Friday. Our deadline is 5pm today so we can assemble the data quickly for discussion tomorrow. Social media was amazing at getting them out: as of 9am today we had 75 orgs/venues returning surveys and over 500 artists.

Feel free to use them as models and adapt them to your situation where you are.

We are working on something in Indianapolis and have released 2 surveys (1 for artists, 1 for organizations) to gather data to help us advocate for the appropriate response specific to our city.
